Terapias complementares: contribuição ou armadilha? 2. Aspectos sobre eficácia
Abstract
Introdução: O relacionamento do médico com as Práticas Integrativas e Complementares em Saúde (PICS) é marcado por ambivalências. Muitos pacientes buscam essas práticas por não se sentirem plenamente acolhidos pelo modelo biomédico convencional, e nem sempre os profissionais estão preparados para orientá-los de forma adequada. Objetivo: Analisar evidências de eficácia das PICS reconhecidas pelo SUS, à luz da literatura científica recente, com ênfase nas implicações éticas para a prática médica. Métodos: Trata-se de uma revisão narrativa. Foram consultadas as bases de dados PubMed e Google Acadêmico, utilizando-se a expressão “controlled trial” combinada com os nomes das 29 práticas reconhecidas pelo SUS, além de “systematic review” como termo adicional. Foram incluídos ensaios clínicos controlados publicados nos últimos dez anos. Para cada prática, foram analisados a natureza da intervenção, os mecanismos explicativos propostos e as evidências de benefícios clínicos reportadas na literatura. Resultados: Os resultados sugerem que diversas PICS apresentam evidências preliminares de benefício em condições como dor crônica, ansiedade, distúrbios gastrointestinais e melhora da qualidade de vida. Embora o grau de respaldo científico varie entre as práticas, algumas delas vêm sendo investigadas com maior rigor metodológico e demonstram efeitos clínicos promissores. Outras ainda carecem de explicações plausíveis para seus mecanismos de ação, o que não impede, contudo, a observação de resultados positivos em determinados contextos clínicos. Conclusão: Com a ampliação das PICS no SUS, cabe aos profissionais de saúde avaliá-las criticamente, com respaldo científico e sensibilidade ética, favorecendo decisões clínicas mais sensatas e centradas no paciente.
